BREAKING NEWS!!!!!!!

The Kidney Disease Program (KDP) Patient Eligibility Information is Now Available Online

Effective immediately, KDP patient eligibility information may be accessed online through the KDP portal. The website to access the portal is www.dhmheclaims.org. Once on the website, click on the “Kidney Disease Program” link. It is necessary to submit a User Agreement to gain access to the portal. This agreement may be found on this website. Simply print the agreement form, complete it and mail it to the following address:

After submitting the agreement, you will receive a user logon and password by email. You will then be able to access the portal and verify patient eligibility and view provider specific claims and payment information. Please note that your computer must have Internet Explorer Version 7.0 or later to access the portal. If you have previously been assigned a portal user logon and password by KDP, you do not need to request another one. The logon information you were assigned will allow you to access the eligibility information as well as check the status of claims.
